Connected Futures Collective was founded on a simple belief: children and families thrive when the people around them work together with shared goals, clear communication, and genuine commitment to what matters most.
Hi, I'm Heather.
I'm an early childhood development practitioner and key worker based in the Clarence Valley, NSW. For over a decade, I've been working alongside children with developmental delays and disabilities, their families, and the educators and allied health professionals who support them.
My work is grounded in a genuine belief that early intervention done well — in real environments, with the people children love and trust — makes a lasting difference. I visit preschools, long day cares, and schools. I sit with families. I collaborate with speech therapists, physiotherapists, and psychologists to build supports that actually hold together.
The 'Collective' in our name isn't branding — it's a commitment. A commitment to shared purpose, genuine partnership, and co-creating futures that belong to everyone involved.

Our key worker and transdisciplinary model means one consistent practitioner coordinates across the team — reducing duplication, confusion, and the exhaustion of managing multiple providers alone.
Understand
We start with a Routines-Based Interview (RBI) to hear what's actually happening in your family's day-to-day — not just what the paperwork says.
Collaborate
We build a plan with your family and the people around your child — sharing goals, strategies, and observations so everyone is working together.
Adapt
Goals are reviewed and adjusted regularly. As your child grows and circumstances change, so does the support.
